By completing this purchase, I consent to the collection and testing of my specimen(s) for DNA and/or drug screening purposes. I authorize the release of results to designated parties, as applicable, and acknowledge that all testing will be conducted in compliance with applicable laws and confidentiality standards.
Cancellation & Refund Policy
All DNA test appointments are non-refundable. Clients may reschedule once within 48 hours of the initial appointment at no additional charge. No-call/no-show appointments are not eligible for rescheduling and will require booking a new appointment at full cost.
